Assumption of Risk: I hereby certify that my child/children is/are in good physical condition and able to participate fully in all VRPlayin/VRKidz program or class for which she/he/they is/are registered.

Release of Liability: I hereby release VRPlayin/VRKidz and its staff from liability in case of accident or injury howsoever caused. VRPlayin/VRKidz is not responsible for any loss of or damage to personal property.

Medical Emergencies: All current medical conditions and those requiring medication have been disclosed on the registration form. I understand that in the event that of a medical emergency, a representative of VRPlayin/VRKidz will notify the emergency contact person as soon as possible at the phone numbers I have provided. In the event that the emergency contact cannot be contacted to provide consent, I hereby authorize a representative of VRPlayin/VRKidz to give consent for any medical treatment that may be required for my child. I understand the representative of VRPlayin/VRKidz will use his or her best judgment in obtaining such service for our child and that I will be responsible for any reasonably related expense.

Media Release: VRPlayin/VRKidz has my permission to use photos and video of my child/children, taken during the camp, on the school's website at vrplayin.ca and for any promotional material and social media websites (such as YouTube, Twiiter, Facebook, and Instagram) related to VRPlayin/VRKidz. I understand that these images will not be given or sold to any outside individual or organization.

Cancellations/Refund Policy: No Refunds for Cancellations within 7 days of the camp starting date. Cancellations received prior to 7 days and all modifications are subject to a $25 administration fee. No refunds will be given for days when a camper is absent, and missed days cannot be made up. A refund due to medical reasons may be considered if a written request is received within a week of the missed session and is accompanied by a doctor’s note.

Drop-off/Pick-up Policy: You must sign in your camper(s) in the morning when you drop them off and sign them out again in the afternoon. Photo ID must be presented each day before your camper can be released from our care. The name on the ID must match the name provided in the parent/guardian 1 and 2 fields or in the notes field on your camper’s application. Please make sure the names you provided exactly match the names appear on your photo IDs. We can accommodate early drop off starting at 8:30am for $12.50 and late pickup until 4pm for $25. Any time after 4pm will be billed at $1 per minute.

The parents/guardians further maintain that the camper(s) is/are the appropriate age for the program(s) requested.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I HAVE HAD SUFFICIENT OPPORTUNITY TO READ THIS ENTIRE DOCUMENT. I HAVE READ AND UNDERSTOOD IT, AND I AGREE TO BE BOUND BY ITS TERMS.

Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating camper/minor (those under 18 years of age) and agree that they and the camper/minor are subject to all the terms of this document, as set forth above. I certify that I am the Parent or Legal Guardian of the above camper/minor.
